The following must be received before acceptance and placement can be made:

_____ Application fee of $185 per applicant, submitted with the online application
_____ Parish Verification of Active Membership form (for those applying for Catholic tuition rate)
_____ Copy of Baptismal certificate (for Catholic applicants)
_____ Copy of state-issued birth certificate
_____ Current and complete Georgia Certificate of Immunization Form (GA3231)
_____ Current and complete Georgia DHR Eye, Ear and Dental Exam Form (GA3300)
_____ Request for Release of Records form submitted to current and prior schools for records
_____ Student Recommendation Form submitted to current teacher through the online application
